Community Habilitation LIFE Counselor

Full-time / $15.00 per hour

The purpose of this position is to provide support to Community Habilitation participants in Onondaga County. These individuals are in need of guidance to complete independent living skills such as shopping, cleaning, exercising and participating in community recreational events.
The successful candidate will offer patience and structure 35-40 hours a week.

Essential Functions:

  • Follow written and/or verbal instructions, including Community Habilitation Plan
  • Complete and submit service documentation / time sheets as directed
  • Adhere to all program and agency procedures
  • Attend all mandatory meetings (trainings, program updates, etc.)
  • Uphold the Codes of Ethics and Core Competencies
  • Exercise sound judgment as a matter of course and hold protected health information and other sensitive information in the strictest of confidence in accordance with LAUNCH and HIPAA policies
  • Other duties as assigned

Requirements:

  • Qualified applicants must pass all OPWDD background checks including fingerprinting
  • Willing to transport participants in Onondaga County in own vehicle, must possess a clean NYS driver's license Must have a working telephone number and access to the internet
  • Basic computer knowledge
  • High School diploma required, associate degree or higher preferred
  • Relevant experience in OPWDD field
  • Ability to work a flexible schedule
